Hey everyone. I just recently received bought this mask and was looking for any background info on it. I dont think the person I bought it from made it. They may have just modified it some? Or made a copy of one they had? (I’m not too familiar with mask making) So I figured what better place to ask then here. I’ve searched for “Boogeymanmasks” in the forum and it seems this may be from a mask maker from some time ago.
On the back I can make out the following:

Boogeyman Masks was still making masks around 2012-2013, had some original sculpts, some masters he bought, and some he modified. I believe the Tiberius originated as a CGP Nemesis mold that he re-sculpted Kirk eyes on. The mask you have seems to be converted from that. I believe his (Boogeyman Masks) name is David Pope, so his signature may be in the mask.
