It’s last known appearance was in late ‘88 at a charity event. As far as I am aware, it has not resurfaced since.

This is not a shot from a scene that was never filmed. They filmed the some of the opening transfer sequences at the Utah State Prison and it was common practice that each time a film was shot there, various actors would get a fake booking photo shot and placed on the wall. They had an unknown crew member don the mask for this shot

Not sure! I asked a contact who worked on the movie, hoping they may know. The stretch is given to the mask several times in the film. Most notably in Bucky’s death.
